ATHLETIC PHYSICALS BISD is hosting two opportunities for students involved in athletics, cheerleading, band, or drill team to get a physical for next school year. Open to all incoming 7th-12th graders. April 17th @ Rudder High School Field House May 15th @ Bryan High School Field House 1: TIME 4:00pm - 5:30pm 2: COST $20 3: WHAT TO BRING Physical form with medical history completed and signed by parent/guardian $20 Glasses or contacts if necessary 4: ACTION PLANS Students with the following conditions will need an additional action plan to be completed by their primary doctor. Diabetes Seizures Allergies requiring an Epi-Pen Asthma 5: VACCINE CLINIC BISD Nursing Services will be on-site hosting a low-cost vaccine clinic. All School aged children may come get their vaccines required for school. 6: FORMS Please click on the following link to access the following forms. 7: ONLINE PARTICIPATION Online participation forms are required before participation next school year. They are available at the link above. It is not required that they be completed prior to getting a physical, but it is encouraged so that their completion can be verified on the physical days.
